Please read these before submitting. They exist so there are no surprises for you or for us.

Scope of the repair

We repair only the service you request and approve. A repair does not include an inspection of the rest of the device. Example: if you request a screen replacement, we replace the screen; we do not test the microphone, cameras, speakers, buttons, charging port, battery or any other component.
Other components may already be faulty before you bring the device in, and we have no way of knowing unless they are tested. MasterPhone Utah is not responsible for any component we were not asked to repair or test, and a fault discovered later is not evidence that we caused it.
A full device test is a separate service. If you want every component checked, ask for a Full Diagnostic before or during your repair and we will quote it. Without it, only the requested repair is covered.
Electronic devices can have hidden or pre-existing damage (prior drops, liquid exposure, previous repairs, aged parts). Such damage can surface during or after any service, and MasterPhone Utah is not liable for pre-existing conditions or for parts that fail because of them.

Warranty

Replaced parts carry a lifetime warranty against defects for as long as you own the device. Labor on repairs that do not involve a replaced part (cleaning, board work, data recovery) is warranted for 30 days from pickup unless otherwise specified in writing.
The warranty covers only the specific part we installed and the labor we performed; it does not extend to other components, other faults or new damage.
The screen warranty is void if the screen is scratched, cracked, chipped, bent, or shows any sign of liquid exposure or impact.
The warranty is void if the device is opened, serviced or tampered with by anyone other than MasterPhone Utah, or if any warranty seal or sticker is broken or removed.
The warranty covers repair or replacement of the part only and does not include refunds of labor, incidental costs, or any claim of loss of use, lost income or lost data.

Data & device condition

MasterPhone Utah is not responsible for any data loss. Please back up your device before leaving it with us. We do not guarantee that photos, contacts, apps, saved games or any other data will be preserved.
Water resistance is not guaranteed after any repair, even if the device was water-resistant when new.
Devices with prior liquid damage may fail at any time, before, during or after service, and such repairs are accepted on a best-effort basis with no warranty on the outcome.
Please remove SIM cards, memory cards, cases and personal accessories. MasterPhone Utah is not responsible for accessories left with the device.

Authorization, pricing & pickup

The customer authorizes MasterPhone Utah to open, service and test the device as needed to perform the requested repair, and confirms they are the owner of the device or authorized by the owner to have it serviced.
Diagnostics are free. If you decline the repair after diagnosis, there is no charge for the diagnostic.
Any price you enter on this form is subject to our verification. The price that applies is the one MasterPhone Utah confirms; you will be notified of any correction before work begins.
Some repairs require ordering parts, and timelines given are estimates, not guarantees. We will keep you informed of any delay.
In rare cases a device cannot be repaired or a fault is beyond economical repair. If so, we return the device at no charge.
Devices not claimed after 30 days may incur storage fees. After 3 months MasterPhone Utah will consider the device abandoned and is not responsible for the device or its integrity.
